What You Should Know About Pistachio Allergies

A pistachio allergy is an immune system reaction to proteins in pistachios. When a person with a sensitivity eats pistachios, the immune system incorrectly identifies the proteins as being harmful and triggers an immune response. Even a small amount of pistachio can trigger this reaction, and the subsequent symptoms can range from mild to severe.

Pistachios are classified as tree nuts and share similarities with other nuts, including cashews and walnuts. There is a particularly close relation between pistachios and cashews, and many patients react to both. Those with a family history of allergies, asthma or eczema are typically at higher risk of developing a pistachio allergy. Patients diagnosed with pistachio allergies are often tested for additional tree nut allergies to reduce the risk of future allergic episodes.

Pistachio allergies can affect adults and children, though they are more likely to develop in childhood. While some children outgrow their allergy, most require ongoing allergy management to prevent reactions. At Safe2Bite, we can diagnose an allergy to pistachios and then create a treatment plan for lasting relief.

Signs and Symptoms of a
Pistachio Allergy

Pistachio allergy symptoms can be unpredictable. For instance, they may start out being mild, then progress to moderate or severe reactions after repeated allergen exposure. As part of your tree nut allergy testing and treatment, we evaluate your symptoms to better understand how your body is reacting. Here are some of the common signs and symptoms of a pistachio allergy.

Swelling of the lips, face, tongue or throat.

Hives or skin
rashes.

Nausea, stomach pain, vomiting or diarrhea.

Itching or tingling in the mouth or lips.

Coughing, wheezing or breathing difficulties.

Nasal congestion or a runny nose.

Dizziness, lightheadedness or fainting.

Anaphylaxis, the most serious reaction to a pistachio allergy, is potentially life-threatening and requires immediate treatment with epinephrine. Families should work with a board-certified allergist to develop an emergency action plan to recognize signs and symptoms of anaphylaxis and to clearly understand when emergency care is required. Both children and adults living with food allergies, such as a pistachio allergy, may experience food anxiety.

Common Sources of Pistachios

Pistachios are found in a range of food products that can include ice cream, gelato, pastries, cakes, cookies, bread, chocolate and candies. These goods may contain whole nuts, chopped pieces, or may be used as a topping. For someone with a pistachio allergy, even trace amounts can trigger a reaction.

Along with more obvious food items, pistachios may go unnoticed when used as a flavoring in certain recipes. Sauces and specialty dishes may include pistachio in the form of a paste or butter, even when it is not the primary flavor or ingredient. It’s also important to watch for cross-contact, which can occur when pistachios are prepared in the same space as other foods.

Families managing a pistachio allergy can reduce the risk of exposure by reading ingredients labels carefully. When dining out, ask about tree nuts, and also inform your child’s school about the allergy.

In the U.S., tree nuts are among the major food allergens that must be declared on packaged food labels. Precautionary statements on labels such as “may contain tree nuts” should not be ignored by people who have pistachio allergies.

Testing for a Pistachio Allergy

Diagnosing a pistachio allergy requires a professional allergist with the ability to accurately interpret results. Even if you feel confident that you understand the nature of your or your child’s allergy, it’s important to get confirmation to discover if other tree nuts could trigger symptoms. Allergists use a range of techniques to diagnose allergies, starting with a consultation that looks at symptoms, medical history, along with family history of allergies. Using this information we can then determine the best allergy testing option for you.

Skin Prick Testing

A skin prick test is a common testing procedure performed at our DFW locations. After your allergist has assessed your reaction to allergens on the skin, the results are usually available during the same visit.

Blood Testing

Blood testing is a good choice when skin prick testing is not possible. A blood sample is sent to a laboratory to see how the immune system responds to pistachios by measuring IgE antibody levels. Typically, allergists will conduct skin testing and blood testing to evaluate food allergies.

Oral Food Challenge

An oral food challenge is a highly accurate test where patients consume small amounts of pistachios under medical supervision. Allergists use it to confirm or rule out an allergy.

Managing and Treating a Pistachio Allergy

At Safe2Bite, we work with families to manage pistachio allergies, reduce the risk of reactions and improve quality of life. We offer a range of food allergy treatment options, including innovative therapies that reduce sensitivity to pistachios over several months. We also ensure that every patient has an emergency action plan in place to be thoroughly prepared if a reaction should occur.

Immunotherapy (OIT)

OIT can help the immune system become less sensitive to pistachio proteins through gradual exposure. Patients consume controlled amounts of the allergen to build immune system tolerance. By desensitizing the body to pistachio protein, you’re less likely to respond to accidental exposure.

Food Sublingual Immunotherapy (SLIT)

SLIT also trains the immune system to tolerate pistachios through gradual exposure. This at-home treatment involves taking daily allergy drops under the tongue. When patients stay consistent this treatment can help lower the risk of reactions from accidental exposure.

When to See an Allergist for a Pistachio Allergy

Parents and families that suspect a food allergy is responsible for symptoms are often unsure of how to proceed. When symptoms are mild or the source is unclear, it’s common to delay seeing an allergist. However, an evaluation can provide a definitive diagnosis and help guide the right pistachio allergy care.

Visiting our food allergy treatment centers is also worthwhile when you don’t know which nuts are safe to eat. For instance, some people are allergic to only one tree nut, while others react to many. The risk of severe reactions can also be higher if you have other food allergies, asthma or eczema.

Finally, if your child has been diagnosed in the past and you suspect they have outgrown their allergy, testing can deliver certainty. Rather than reintroducing pistachios at home without medical supervision, we can perform safe allergy testing to see how the allergy has changed.
