Understanding Sesame Allergies

A sesame allergy is an immune system reaction to the proteins found in sesame. When you’ve eaten foods containing sesame, your immune system sees those sesame proteins as a threat, then releases IgE antibodies to protect the body. Usually within a few minutes of eating the food you can experience allergic symptoms ranging from mild to severe, and even life-threatening in the most severe cases.

Sesame allergies are most common in children and often begin in early childhood. Most sesame allergies appear between 6 months and 3 1/2 years of age, though they can develop at any age. While some children do outgrow their allergy, this occurs in only about 20–30% of cases. The remainder of sesame allergy sufferers will require ongoing care.

As part of recent FDA Food labeling requirements, all packaged foods must include identification of sesame as a major allergen. However, this does not eliminate the possibility of exposure via unknown ingredient names and/or through cross-contamination in food preparation.

of a Sesame Allergy

Sesame allergy symptoms usually occur quickly after exposure, though they can sometimes be delayed by several hours. Symptoms vary from person to person and may affect different parts of the body. In some cases reactions can be severe, including anaphylaxis, a life-threatening condition that requires immediate medical attention. Here are the sesame allergy symptoms to watch for.

Swelling in lips, face, throat or eye area.

Stomach pain, nausea, diarrhea or vomiting.

Itching or tingling in the mouth or throat.

Coughing or wheezing.

Hives, welts or itchy skin reactions.

Dizziness or fainting.

Chest tightness or difficulty breathing.

Foods That Contain Sesame and
How to Avoid Them

Like many common allergens, including peanut, milk and tree nuts, sesame can appear in unexpected products. While some sesame-containing items will be obvious, others could contain hidden ingredients that require close inspection of food labels. If you or your child have a sesame allergy, it’s important to get into the habit of checking ingredient lists, informing the school of dietary needs, and asking questions at restaurants.

Common food sources containing sesame include breads, bagels, crackers, hummus, tahini, salad dressings and many snack foods. Sesame can also be found in oils, pastes and spice blends. When dining out, dishes featuring stir-fries, dips, flatbreads or marinades commonly contain sesame, so always check with your server.

Another important factor is the potential for cross-contact during food preparation at home or in restaurants. Unfortunately, even trace amounts of sesame on shared utensils or surfaces could trigger a reaction.

Families managing sesame allergies need to coordinate efforts with schools, daycares, relatives, and even restaurants to minimize risk of accidental exposure. A comprehensive plan for education and preparedness is critical to effective long-term management of a sesame allergy.

Sesame Allergy Diagnosis and Testing

Getting tested for food allergies is the most reliable way to identify your allergy triggers and confirm a diagnosis. During the evaluation, our board-certified allergist will review your symptoms, identify the foods eaten before a reaction, and consider any family history of allergies. This process can reveal patterns that point to a sesame allergy and rule out other possible causes. After this initial evaluation, we may recommend one or more of these food allergy testing procedures.

Skin Prick Testing

A skin prick test is a quick and minimally invasive option for confirming a sesame allergy. Redness or swelling at the test site indicates a likely sensitivity.

Blood Testing

Blood testing measures IgE antibodies to sesame in the blood. After a standard blood draw, the sample is analyzed in a laboratory and the results are interpreted by an allergist.

Oral Food Challenge

An oral food challenge (OFC) is the most accurate diagnostic tool available for confirming a sesame allergy. Allergists may recommend an OFC when results from other tests are inconclusive.

Treatment for Sesame Allergies

Managing a sesame allergy typically includes a combination of avoidance strategies and advanced food allergy treatment options. While strict avoidance has traditionally been the standard approach, newer therapies are helping patients better manage their allergy. Along with carrying epinephrine auto-injectors in case of emergency, desensitization treatments can help reduce sensitivity to sesame.

When it comes to food allergy treatment, some of our patients’ goals include minimizing anxiety related to food allergies and living comfortably without having to worry about accidental exposure at work, school, restaurants and other social settings, or during travel.

Oral Immunotherapy Treatment (OIT)

Oral immunotherapy is a method for gradually building tolerance to sesame. Over several months, you consume small amounts of the allergen under the supervision of a board-certified allergist, and continue until you reach a maintenance level. By retraining the immune system you will increase your tolerance to your allergic food, thereby providing protection.

At Safe2Bite, the sesame OIT program is provided by highly-trained and experienced allergists, with treatment tailored to each patient’s unique needs based on age, allergy history and degree of sensitivity. Regular follow-up visits allow us to monitor the progression of treatment and assure you that treatment is safe and successful.

Food Sublingual Immunotherapy (SLIT)

Sublingual immunotherapy involves placing allergy drops under the tongue to gradually build tolerance to sesame. Using low doses of sesame allergen, SLIT can be administered at home, with periodic visits to our allergy treatment centers for evaluation.

We may recommend sesame SLIT for some patients as an alternative to OIT. Based on the severity of your allergies along with your lifestyle and treatment goals, we will work together to determine which treatment method is most suitable for you.

For many people, including parents of young children, knowing when to visit an allergist may not be obvious. But when you have any concerns about a potential allergy, it’s always worth speaking with an allergist sooner rather than later. Paying close attention to symptoms that develop after eating sesame or other foods is the best way to identify an issue, particularly when reactions are a recurring event.

Families should also visit one of our allergy treatment centers when they need updated testing results. As some children will outgrow their allergy over time, a food allergy test can provide guidance and ensure that foods are not unnecessarily avoided.
